Sally Forman, 92, went to be with the Lord on Saturday, May 7, 2016. Her mind cleared and she went to meet her husband, Joe Forman; her sister, Nellie Krno; brothers, Mike, Joe and Wally Milchanowsky; and her parents, Mike & Suzie Milchanowsky. The world is a lesser place today. We have lost a most kind, gentle, funny, wonderful person. She can now rest in peace. She is survived by her son, Eric Forman; two daughters and sons-in-law, Lorraine & Peter and Connie & Dan Chambers; a granddaughter, Gaby; grandson, Craig, and his wife, Monica; and several nieces and nephews. The family would like to offer heartfelt thanks to Sally's extended family, the nurses, aides and staff who cared for her at Susquehanna Nursing Home. Funeral services will be held at 11:00 AM on Friday, May 13, 2016 at Vestal Hills Memorial Park, Tower of Memories Chapel. Entombment will follow.
